We are glad you are here. We are happy to see you for consultation services only. We WILL NOT become or be your Primary Care Provider. You will keep your present primary care doctor. We will be glad to work together with your present doctor while we provide specialized care.

To start your healthcare with Renew, please download the necessary forms by clicking on the download links to the right. Please complete these forms and return them to us by Fax, Mail, or in person at the office.

Once we have reviewed your completed application we will call you to schedule a Discovery Call. This will allow us to see if we are a “fit” for each other and if we are able to provide the specialized care you deserve.

We are committed to make this a convenient, comfortable place for you to obtain your regenerative medicine services.

Our office provides care for you in an outpatient, or office only, setting. Our focus on outpatient care allows us to provide timely and personalized care for you in the comfort of our office.

Please call to schedule an appointment – We are not a walk in clinic. We will schedule your appointment with you based on acuity and need. We will work to serve you in a prompt and timely manner!

If you are unable to keep your appointment, please call the office at least 24 hours in advance of your scheduled appointment to cancel so that we may accommodate another patient’s needs. We do have a set “no show” fee that will be assessed if you fail to keep your appointment. This fee is NOT covered by your insurance and is your personal responsibility that will need to be paid before another appointment is scheduled.
